Colorado counties are continuing to administer COVID-19 vaccinations to those in Phases 1A and 1B. Appointments are required at most sites. Vaccine providers and updates are available by county on the Colorado Department of Public Health and Environment (CDPHE) website.
If you don’t see your county or provider listed below, visit Colorado’s vaccine provider list, which is organized by county. Additionally CDPHE recommends checking each provider’s website to see if vaccines and appointments are available for eligible individuals and for additional information.
Chaffee County
- As of February 8, CCPH is operating drive-through vaccine clinics Mondays and Tuesdays for those in Phases 1A, 1B.1, and 1B.2. Eligible individuals can make an appointment using the online scheduler: https://chaffeecounty.rsvpify.com/. New appointment slots will be offered as vaccine inventory becomes available. Custer County
Custer County is currently vaccinating those age 70 and older, and they are in the process of contacting those who have completed the online form or called in. Those interested in a COVID-19 vaccine should fill out the county’s online form or call 719-430-4601. The county will contact interested parties with information and to schedule an appointment when eligible.
Denver County
Kaiser Permanente
- Kaiser members and non-members age 65+, as well as those in eligible 1A and 1B categories, can complete Kaiser’s online “COVID-19 vaccine sign up.” Individuals will then be placed on a first-come, first-served wait list for the vaccine based on their eligibility.
Denver Health
- Denver Health patients will receive appointment invitations based on the state’s eligibility guidelines. Patients can sign up or log in to their accounts at www.denverhealth.org/mychart. Instructions on creating a MyChart account are available on Denver Health’s website, and additional details can be found on the Denver Health COVID-19 vaccine website.
- Non-patients can indicate their interest in receiving a COVID-19 vaccine when doses are available by registering on the Denver Health COVID-19 vaccine website or by calling 303-436-7000 (Monday to Friday, 7:00 a.m. to 6:00 p.m.) to register. Both online and phone registration will automatically place you on a vaccine waitlist. Visit COCovidVaccine.org for more information and updates on which groups are currently eligible.
HealthONE
- Due to limited vaccine supply, HealthONE requires appointments to receive the vaccine at participating facilities. Walk-up vaccinations are not available.
- Individuals should complete the COVID-19 vaccine request form to receive additional information and instructions about scheduling an appointment. If you do not have access to email or need additional assistance, please call 720-650-8274 and leave a message with your full name, date of birth, and phone number. Names will be randomly selected from the online and call-in waitlists. Participants will receive an email or a call when it is time to schedule their vaccine.

SCL Health
- SCL Health is contacting patients age 70 and older currently receiving care at an SCL Health facility. Information is being sent to these patients on how to use the MyChart online patient portal to make a vaccination appointment. These invitations are being sent in phases based on the state’s prioritization system.
- Those interested in receiving the vaccine can create an SCL Health MyChart account for the fastest, easiest experience. Visit MyChart.SCLHealth.org or download the SCL Health MyChart app to create an account and schedule an appointment if you are eligible and vaccines are available.
- Appointments are required to receive the vaccine at SCL Health facilities. Walk-up vaccinations are not available. Those 70 and older seeking more information about registering for a COVID-19 vaccine at an SCL Health facility should visit SCL Health’s website or call 303-812-2051.
Eagle County

Fremont County
Centura Health
- Centura Health patients will receive an invitation through their patient portal when they are eligible for the vaccine based on the state’s phased prioritization. The process will be randomized, beginning with those age 70 and older, with no discrimination based on race, color, or ability to pay.
- Patients should use the vaccine notification sign-up form for additional information and instructions via email or text. Patients may not receive immediate correspondence after filling out the form, but Centura may provide updates and scheduling when the vaccine becomes available. Patients are not required to complete the form to get the vaccine.

Lake County
As of February 8, Lake County is vaccinating residents in Phase 1B.2. Lake County Public Health Agency is coordinating COVID-19 vaccination phase scheduling, distribution, and reporting through the CDPHE-approved COVID Vaccination Provider Program for Lake County, as well as St. Vincent Health and Rocky Mountain Family Practice.

Park County
Park County is addressing residents in Phases 1A, 1B.1, and 1B.2. Those interested in receiving the vaccine when eligible should complete the PCPH Pre-Registration Form. Because supply is limited, it may take a few weeks to receive the vaccine.
